An Effective Practice

Description

The North Dakota Diabetes Care Provider Report project is a program of the North Dakota Diabetes Prevention and Control Program and Blue Cross/Blue Shield of North Dakota to improve diabetes care. Through the initiative a registry of patients tracks diabetes care using insurance claim information. Physicians receive quarterly reports that include details of preventative care provided to each of their patients including office visits, A1C tests, eye exams, lipid tests, and microalbumin tests.

Goal / Mission

The goal of the North Dakota Diabetes Care Provider Report is to improve quality of diabetes care and management in the state.

Results / Accomplishments

Quarterly reports were sent to nearly 600 physicians. Physicians reporting that patients received all five preventative care measures increased from 13% at the onset of the program to 45% in 2007. The program has shown a 15% improvement in ambulatory care measures including A1C, lipid and microalbumin tests, and eye exams as well as a 24% decrease in emergency room visits and a cost savings of $530 per patient. The success of the program has led to the development of a project called MediQhome which will expand the initiative to cover a predicted 80% of patients in the state, not just those covered by Blue Cross/Blue Shield.
